H7. Maternal Health Baseline Survey – Assessing Community Decisions Regarding Maternal Health

This tool is a questionnaire to be conducted with women aged between 13 and 49 years who are currently pregnant or have given birth in the last 12 months. The questionnaire aims to determine the mother’s health, if she experienced any complications during childbirth and what she would do/has done if experiencing maternal complications.
